LEAF CHIROPRACTIC SERVICES

Although we’re accustomed to seeing medical services tents at large tournaments, parades and fairs, I believe DYAA is unique in now offering this as a complimentary, on-field service for all DYAA Rec and DSSC Select players. Let me elaborate:

We are very pleased to announce that the Leaf Chiropractic & Wellness Center is a new corporate supporter of DYAA and DSSC. As such, Dr. Guy Phillips will provide the following complimentary, on-field services for any/all DYAA and DSSC players this spring:

∑ Concussion testing

∑ Injury evaluation

∑ Taping and bracing

∑ Chiropractic adjustments

∑ Post-treatment referrals

Dr. Phillips is a certified Chiropractic Physician, a certified Athletic Trainer and a certified Strength and Conditioning Specialist. He will set up his medical services tent, treatment table and supplies as indicated below (although professional circumstances may require modification to his scheduled availability):

April 18th 9a - 1p          at Mingo Park

April 25th   9a- 1p           at Mingo Park

April 29th 6a - 7p     at Smith Park

May 2nd   9a – 1p         at Mingo Park

May 9th     1p - 4p      at Mingo Park

May 16th   4p - 6p      at Smith Park

May 20th 6a - 7p      at Smith Park

May 23rd 9a – 4p     at Mingo Park

If you are playing at a location when/where Dr. Phillips is set up at another park, you can come to the park where Dr. Phillips is located in order to utilize any of the above-named services (i.e. pre-game taping, post-game injury treatment or evaluation, etc).

Prior to receiving services or advice from Dr. Phillips, the player’s parent must sign (or have previously signed) a medical authorization and informed consent form. Your coach will be given a limited supply of these consent forms as well as information about the Leaf Chiropractic and Wellness Center. Dr. Phillips will also have these forms at his treatment tent…and DYAA will keep a supply of informational brochures at the Mingo Concession Stand.
